We are currently open and scheduling patients at this time. We are practicing all necessary precautions by minimizing patient traffic in the clinic, cleaning all surfaces and equipment used, as well as screening all patients that we have in the clinic. If you are scheduled to come in, we are requiring that all patients have a mask or face covering. 


We are currently offering telemedicine for all patients with no copay (depending on the insurance) and look forward to continuing to help as many people as we can! Contact us if you have any questions, would like to set up an appointment, or are interested in any social distancing PT assistance!

Your First Appointment

Our appointments are always on schedule. 

We will ask you to arrive 30 minutes early for your first appointment to complete the necessary paperwork.  If you have completed the Medical History Form through our on-line Portal, and downloaded and filled out the form below (based on your insurance type), you can arrive 10 min before your appointment time.

To save some time, you can to download the demographic/insurance information form that needs to be completed. Please click on one of the following insurance types to download your first appointment forms:

Covered by No-Fault /Injured in an auto related incident 
Covered by Workman's Compensation/ Injured at work 
All Other Insurance Types or Uninsured

What to Bring to Your First Appointment:

- Your insurance card
- Your prescription
- Referral forms if required by your insurance provider
- Pertinent information about your medical and surgical history
- Case/Policy numbers if claiming under No-Fault or Worker's Compensation
- Completed patient information forms, if you have downloaded them. 

Wear comfortable, loose fitting clothing.

You may contact us by phone during our office hours to make or change an appointment.  Please give 24 hours’ notice, when possible, to cancel an appointment. 

Billing

If you have a question regarding billing or insurance coverage (from either office), please call and speak with Gina Cunningham at (716) 662-2922.  She is available Monday through Friday between the hours of 6am and 1:30 pm.
